How To Make An Appointment

Please call our main office number at (561) 845-6000 to schedule an appointment with any of our physicians. Our normal office hours are from 9:00 A.M. to 5:00 P.M. Monday through Friday.

Please note that our staff has lunch from 12:00 P.M. to 1:15 P.M.

Preparing For Your Appointment

• Please arrive at least 15 minutes before your appointment time so you can fill out any necessary forms. Remember to bring any forms that you have filled out from the website. Which can be found below.

• Please bring your insurance card and driver’s license.

• Please bring any medical records or imaging studies (both the actual films and the radiology reports for x-rays, MRI’s and CT scans) if you have been seen for the injury in the past.

• If your insurance coverage requires a referral please be sure to bring the form from your primary care physician or your insurance company.
